Who should not whiten
Patients with active tooth decay or leaking fillings (the peroxide penetrates into the pulp and causes severe pain). Patients with exposed root surfaces from gum recession (roots do not respond to whitening and will remain darker). Heavily restored front teeth — the bleach whitens natural enamel but not the composite or crown, leaving a mismatch. Under-16s, pregnant or breastfeeding patients — wait.
What to expect after
Transient sensitivity to cold is normal and settles within 24–72 hours. Avoid dark foods and drinks (coffee, tea, red wine, curry) for 48 hours while the enamel pores are still open. Expect 3–8 shades of improvement depending on baseline; stop believing photos that show 20 shades of difference — those are lighting tricks.
How long it lasts
Professional whitening typically lasts 12–24 months before a touch-up is worth considering. Smokers and heavy coffee/tea drinkers should expect the shorter end. A maintenance routine of take-home trays every 6 months extends results considerably. Whitening toothpaste used daily can help surface maintenance but should not replace a proper touch-up.
